Below you will find links to the 3 forms that we use for
surveying, booking, and planning the shows that we do. Please feel free to
fill in whatever information you feel important when considering us for an
event. While simply filling in a form is not to be construed as a
contract, please keep in mind that all information submitted to us by
these forms is completely and strictly confidential and will not, under any
circumstance, be circulated outside The
Jim & Lance
Show. It is through the
information given to us on these forms that we obtain almost all the information
we need to map out the events that we do, from a bridal dance to the final song,
from the music we play when certain events take place to special music to have
on hand.
When choosing which form to use, please use the following as a
guideline:
Charity Event - Use this form for a charity event of
any type. (Form is currently under construction.)
Party - Use this form for anything that does not fall
into the first two categories below. If all the information you
require cannot fit into this form, the remainder can be supplied in
subsequent conversations. (Form is currently under construction.)
